Your health information is collected via electronic forms, information systems/electronic forms, online
electronic forms,e-mail and documents declared by you.
What is the Legal Reason for Collecting Special Categories of Personal Data?
Personal data relating to the race, ethnic origin, political opinion, philosophical belief, religion, religious sect
or other belief, appearance, membership to associations, foundations or trade-unions, data concerning
health, sexual life, criminal convictions and security measures, and the biometric and genetic data are
deemed to be special categories of personal data.
Special categories of personal data require higher protection than other data categories. It is regulated in
accordance with the Personal Data Protection Law No. 6698 that additional information should be provided
in order to process such data.
To Whom and For Which Purposes May Your Personal Data Be Transferred Domestically?
● In accordance with the legal regulations and the legislation we are subject to, it is shared with the
people who are clearly authorized in the legislation, especially the Social Security Institution. In
addition, your personal data will be shared with authorized persons or public institutions and
organizations upon the request of a court decision or authorized administrative authorities.
● Your personal data may be shared with our suppliers for the purpose of carrying out supply chain
management processes and fulfilling the contract regarding the products or services to be procured
from suppliers within the scope of the service contract between us and the suppliers.
● Your personal data may be shared with natural persons or private law legal entities with whom we
cooperate for the purposes of executing finance and accounting works, executing contract
processes, and executing investment processes within the scope of the service contract between
us.
● Your personal data may be shared with our business partners with whom we cooperate within the
scope of the service contract between us, for the purpose of conducting business activities and
fulfilling the contract.
Do We Transfer Your Personal Data Abroad?
● Your personal data is not transferred abroad.
How Long Is Your Personal Data Stored?
● If a period of time is determined for the storage of your personal data in the law or the relevant
legislation, your personal data must be kept for at least this period. If a period is not foreseen in the
law or the relevant legislation; It is stored for reasonable periods determined in connection with the
purposes of processing, provided that it is limited and measured.
What are Your Rights with Respect to Your Personal Data as a Data Subject?
Pursuant to your personal data, you have the following rights:
● to learn whether your personal data are processed or not,
● If your personal data are processed, to request information there about,
● to learn the purpose of the processing of your personal data, and whether these personal data are
then used for the intended purposes or not,
● to learn the identity of third parties to whom your personal data are transferred within country or
abroad,
● to request the rectification of the incomplete or inaccurate data, if any,
● to request the erasure or destruction of your personal data within the frame of the terms and
conditions specified in Law,
● If you request completion or correction of incomplete or inaccurate data, or erasure or destruction
of your personal data, to request that this situation be notified to the third parties to whom we
transfer your personal data,
● to object to the emergence of a result against the person her/himself due to analyzing the processed
data exclusively through automatic systems,
● to request compensation for the damage arising from the unlawful processing of your personal
data.
How To Exercise Your Rights As a Data Subject?
You can forward your applications and requests regarding your personal data to Lösev Vakfı via the Data
Subject Application Form by;
● By sending a wet-signed application accompanied by a photocopy of your identity card, to the
address of Turgutlu Sokak, Büyükesat Mahallesi, No:30 Çankaya/Ankara
● By filing an application personally with a valid identity card,
● By sending it to the registered electronic mail address of …… via using registered electronic mail
(REM) address and secure electronic signature or mobile signature.
● By sending an e-mail with signatured peition by Data Subject with mobile signature or secure
electronic signature or by filling “Data Subject Application Form” to kvkk@losev.org.tr address.
In accordance with Communique On The Principles And Procedures For The Request To The Data
Controller, the application filed by the Data Subject is required to contain such information as first name,
surname, signature (if the application is in writing), T.R. identity number (if the applicant is a foreigner,
nationality, passport number, or if any, identity number), residential or office address for notification
purposes, and if any, electronic mail address for notification purposes, telephone number, fax number, and
information about the subject of request.
The Data Subject shall expressly and understandably state the matter requested in the application,
including the explanations regarding the exercise of the rights mentioned above and the rights that she/he
requests to exercise. All information and documents related to the application should be attached to the
application.
Although the subject of the request must be related to the person who makes the applicant, if acting on
behalf of someone else, the applicant must be specifically authorized in this regard and this authorization
must be documented (letter of proxy). In addition, the application is required to contain identity and address
information, and documents of proof of identity are required to be attached to the application.
Requests filed by unauthorized third parties on behalf of someone else will not be taken into consideration.
How Long Your Requests Will Be Answered Regarding Your Processed Personal Data?
Your requests regarding your personal data are evaluated and answered as soon as possible and within
30 days at the latest from the date they reach Lösev Vakfı. If your request is accepted or rejected by
explaining the reason, then, our answer will be sent to the address you specified in the application, primarily
by postal or e-mail, if possible, in the same manner as your request.
